Department of Pharmacy
The Department of Pharmacy is a part of the clinical support services which embodied as one of the aspirations of the long-drawn-out development of Sultan Ahmad Shah Medical Centre @IIUM (SASMEC @IIUM), Kuantan. Established in July 2016, the Department of Pharmacy provides quality services of both inpatient and outpatient care, as well as effective pharmacy practice through optimal individualized pharmaceutical care to patients. The Department of Pharmacy is also responsible for the procurement of drugs and consumables and plays a vital role in promoting rational, cost-effective drug usage through an efficient delivery system. There are about 25 pharmacists and 46 pharmacy assistants who form a close-knit, friendly working environment. The department also builds a good rapport with the Kulliyyah of Pharmacy, IIUM Kuantan Campus whereby two consultant pharmacists were based in the hospital, providing a rich and stimulating diverse environment.

Inpatient Pharmacy provides services to the wards, clinic and other units in the hospital through the unit of use and stock top up system. For unit of use, each dose is packed individually and supply every 2 or 3 days according to the schedule. Psychotropic and dangerous drugs are also supplied to the wards and units from Inpatient Pharmacy.
Inpatient Pharmacy caters inward supply of medications. Medications are supplied to the patient admitted in the ward via prescriptions ordered by doctors. The supply is made through medication trolley every 2 days and also via prescription order for urgent need medications.
Satellite Pharmacy provides services to the inward patient who is ready to be discharged. All matter relating to discharge procedure, bedside dispensing and charging procedures are catered here.
Satellite Pharmacy which is located at level 7 supply medications for discharged patients, bedside and discharge counselling during office hours only. After office hours, weekend and public holidays, all services will be handled back by Inpatient Pharmacy. Patients will collect medications at Emergency Pharmacy.

Outpatient Pharmacy caters patients who are from clinics, daycare and emergency department (during working hours, i.e. 8.00am – 5.00pm). The patients who are on chronic medications which require more than 1 month of medication will be required to refill their prescription at Outpatient Pharmacy. Medications will be supplied according to the current stock status.
- Supply medications for outpatient through prescriptions ordered by medical officers from hospital outpatient services.
- Provide medications counselling for newly prescribe medication device and reassessment for poor compliance patient.
- Provide medication therapy adherence clinic (MTAC) services.
Emergency pharmacy operates from 11.00pm until 7.00am just to supply simple discharge medications to patients discharged from the Emergency Department. In view of minimal medications kept in the Emergency Pharmacy, patients may require to return to Outpatient Pharmacy during working hours for medications that are unavailable. Emergency Pharmacy will not entertain prescriptions from clinics. All prescription by clinics is to be taken at Outpatient Pharmacy.

The Production Pharmacy Unit of Pharmacy Department supports SASMEC @IIUM services by supplying aseptic preparation as well as non-sterile preparation. Aseptic Division of this unit provides three components of services; Cytotoxic Drug Reconstitution (CDR), Parenteral Nutrition (PN), and IV Admixture & Eye Drop Preparations. All these are prepared in Certified Clean Room by our trained personnel.
The production Pharmacy Unit is also responsible for non-sterile products to be repacked into smaller packaging. The repacking process will follow strict Good Production Practice (GPP).
Our cleanroom is divided into four grades; Grade A, Grade B, Grade C and Grade D, which A is being the cleanest, and D is being more lenient. These grades are classified and have to adhere to several criterias, such as the maximum amount of viable microbes per meter cube, maximum amount of particles per meter cube, type of air filter used and its efficiency in doing filtration, and air-change rate per hour – which all these will define the grade of the area and the type of preparation or compounding that can be done under that area.

Clinical Pharmacokinetics Service (CPS) is an extension from pharmacy service that plays important role in patient care and individualized therapy. In order to provide efficacious yet safe therapeutic management to the patient, both Pharmacokinetics (PK) and Pharmacodynamics (PD) principles are used in predicting individualized treatment response.
The main service provided by CPS Unit is Therapeutic Drug Monitoring (TDM) – a branch of clinical chemistry and clinical pharmacology that specializes in the measurement of drug concentrations in blood. This service focuses in monitoring the drugs that have narrow therapeutic index, where a small difference in dose or drug’s concentration can be easily under- or overdosed, thus may lead to either treatment failure or toxicity.
To achieve safe and appropriate therapeutic goals for patient especially in management that involves Narrow Therapeutic Index (NTI) drugs.

The Medical Store Unit is the heart of SASMEC @IIUM which serves as the central procurement, receipt, storage, supply and distribution of medical items to other departments.
The main objective of the unit is to manage and control medical items inventory in order to ensure that the right quality product, in the right quantities, and in the right condition is delivered to the right place, at the right time. Apart from that, all processes involved in the unit emphasises on the safety of medical items as well as the employees.

The main service of Drug and Poison Information Services (DIS) is to answer any inquiries about medicines received from other health professionals as well as the general public. Other services provided by this unit are as follows:
- Medication Assessment: Journal search, evaluation of clinical studies, data preparation for drug assessment and advice on the potential role of new medicines in treatment.
- Information Dissemination: To disseminate the latest information and pharmacy bulletins to all residents of the hospital. We also provide advanced search service by using and advise on primary, secondary & tertiary resource options.
- Management of Medicine Formulary: Manage of SASMEC @IIUM Medicine Formulary, Secretariat of Drug and Therapeutics Committee (DTC) and evaluate Special Formulary Drug Request.
- Pharmacovigilance: Adverse Drug Reaction (ADR) Reporting, Allergic Reaction Reporting, product complaint, Incident reporting and Medication Error (ME) reporting.
- Provide Training & Continuing Education: Engage in education and training for all levels of pharmaceutical staff, organize/prepare materials for discussion and assist consumer education from the drug company
